I am currently on Methyl B12 (5 mg) and Folinic (0.8 mg) in a 0.3 ml subcutaneous injection as a treatment for a homozygous C677T gene mutation. I have recently become aware that Folinic isn't the most active form of B9 available. My understanding is that this product does include the most active form of B9. Is there a difference between Metafolin®* L-5-methyltetrahydrofolate and Metafolin L-Methylfolate? I'm guessing there is.
I am considering purchasing this product to try it out; however, do I need to stop the injections before trying this or are the levels safe enough that I can just try this in addition to see if I feel a difference then worry about stopping the one I don't think is helping.

Emily - Hi Emily - If you are using Folinic Acid for a homozygous C677T MTHFR mutation, that is not doing you any good in terms of bypassing the problem. Folinic acid is a few steps above methylfolate and the MTHFR enzyme works one step above methylfolate. This means that folinic acid is not doing anything. I do recommend you talk with your doctor about stopping the injections. Since this may be the first time you are taking methylfolate, I recommend you take a 1/4 tablet first and see how you feel. Increase by 1/4 each day and see how you feel. If you feel symptoms of the following, then you are taking too much methylfolate: - irritable - depressed - muscle pain - sore joints If these occur, simply stop taking the methylfolate for a couple days and then reduce the amount you were taking back to the amount you were tolerating. With a homozygous C677T MTHFR mutation, I am thinking you will be able to tolerate 2 full tablets a day quite well - but again - work up to this amount. Since this is a sublingual supplement, the methylcobalamin (active B12) is very well-absorbed so you likely won't need to continue injectable B12 anymore - but...this is for you and your doctor to discuss. - Hello - Sublingual Active B12 with Methyfolate will help those with A1298C and C677T MTHFR mutations. It also will help those who simply have macrocytic anemia. The tablets dissolve readily in anyone's mouth as long as there is some saliva present. Those with very dry mouths are suggested to take a sip of water, juice or tea and swallow. Then place the tablet in the mouth. Optimal Multivitamin is recommended alongside Sublingual Active B12 with Methylfolate because of all the complementary B vitamins, minerals and antioxidants. They were prescribed by a hematologist and I purchased the exact amounts she told my daughter to take.She is seeing a high risk perinatologist, but I am concerned that she isn't taking any supplements...he may put her on them later but what about now??

I have a few 'beginners' questions. I'm just beginning to address my MTHFR (compound hetero C677T/A1298C) and started with the following:

- Hello - Your choices look pretty good. Just keep in mind you don't over do it. Watch for signs/symptoms of methylfolate excess. I am not sure about the Concentrace effect. It is not really an electrolyte drink - it is more a complete mineral drink - and it is also foul tasting ;) You can definitely try it. I'd wait 20 minutes to start drinking the MTHFRade post methylfolate and methylcobalamin. If you have methylfolate and methylcobalamin as sublingual forms, then the likelihood of damage to either of them is minimized. I am homo for C677T mutation. I am also quite sure that I have pyroluria. I am just starting to treat myself since I can"t find a doctor that is knowledgeable on mthfr. I have a concern about the mannitol in the sublingual B12 since I have never done well on any of the -itols. Especially zylitol in even the smallest amounts-my stomach blows up. Should I stay away from the sublingual and go with the homocysteX or since my gut issues are probably due to a b12 deficiency maybe I wouldn't be so sensitive as I increase my b12? Also, will the Optimal multivitamin have enough B6 and zinc to treat pyroluria or do I need to add them individually to my protocol ? I also have MS (or so they say). Thank you Dr. Ben! Hello -
You may do best on the HomocysteX to start. Simply take 1/2 to 1 capsule on an empty stomach for a week to see how you feel. Increase by 1/2 to 1 capsule every week until you feel quite good. Once you feel 'off' or not right, go back to the amount where you did feel good.
I highly recommend you obtain a bottle of extended release Niacin and Neutralize to help combat potential side effects from HomocysteX or any supplement containing methyl groups.
Optimal Multivitamin does not have enough zinc or B6 to help with pyroluria. I definitely recommend testing for pyroluria before you begin taking supplements for it - especially high dose zinc and B6 as both these nutrients are not safe in high amounts long term.
The B6 found in Magnesium Plus is in the form of P-5-P which is the most active form and also in quite a potent amount.
Since your stomach is also sensitive, I recommend you consider taking ProBiota Sensitive - 1 capsule after dinner - along with 1 capsule of Saccharomyces Boulardii. These are very pure and effective strains of probiotics which may help balance your digestive fllora.

- I've just been diagnosed as compund heterozygous MTHFR 677 and 1298. I've recently started on your multi vitamin, b12/metafolin, and add'l support like active b6 and molybdenum. I've also just tested positive for pyroluria--it was recommended I take 50 mg zinc plus 5 mg copper because it's low too--not sure if this is enough zinc. My children have not yet been tested but I suspect these problems in them as well. My children are almost 6 yrs and 42 lbs and almost 3 yrs and 32 lbs--what would the correct doses of b12/folate, b6, and zinc be? I'm planning to start them on your chewable multi as well. Thanks.
- Hello - With pyroluria, it is good to taper up in the recommended deficient nutrients - such as zinc, manganese, b6. There is no set appropriate dosage for pyroluria for children or adults - it is individualized and must be attended to in order to not over supplement with zinc or b6 or copper. B12 and folate levels are also individualized and should be tapered up from a small amount to the amount that works best for you and your children. I have heard that those with pyroluria can take upwards of 50 mg of zinc twice daily - however, again - this is not a suggested dose for you or your children because everyone is different. I am on supplements of magnesium and take shots of B12 now and because of my migraines, I also get infused with magnesium along with very potent steroids. I am concerned that my body is rejecting all the nutrients because I cannot metabolize them. This has been a life long issue and am finally finding answers thanks to Dent Neurological center and your website!! However, this is still so very confusing and I would like to feel better as soon as possible. I am 45 years old and have most if not all of the symptoms for each of these problems and am getting more worried by the minute.....any information or help you can give would be so greatly appreciated. Deplin is a great medication for those needing high amounts of methylfolate to bypass folate receptor antibodies and in some folate mutations.
It depends on the person if Deplin is suitable or not. I do not have enough information to say one way or the other if less methylfolate will help or hinder you.
Deplin is very strong and it only addresses the methylfolate. If you are low in B12, then the Deplin won't be doing much good.
I always say and recommend people that you must start with the basics and work out from there - this means lifestyle and dietary changes first.

Without laboratory testing, physical exams and history, I cannot make a judgement call if Sublingual Active B12 with Metafolin is safe for you during pregnancy.
The typical recommendation for pregnant women regarding methylfolate is 800 mcg.
There is no typical recommendation really for methylcobalamin - some women need more and some less. Your doctor has to make that recommendation.
These tablets are easily broken into smaller pieces should your doctor want you to take less.
